I voluntarily agree to the Pastoral Care provided by Living Water Church. I understand and recognize that such Pastoral Care consists of guidance about ideals, beliefs, and Scripture as interpreted and ministered who has completed the training established by SYMBIS (Saving Your Marriage Before It Starts) and is a Certified SYMBIS Facilitator.
I recognize and understand I am not receiving secular or psychiatric, psychometric, or psychological coaching.
I understand and recognize that a Pastoral Care session is not a licensed mental health care provider. Our Pastoral Care sessions is not holding himself/herself out as a "psychiatrist," "psychometrist," or "psychologist," nor is his/her guidance considered "psychiatry," "psychometric," or "psychology."
I acknowledge and understand that the Pastoral Care received from Living Water Church does not and will not assess, diagnose, correct, treat, or coach in a professional relationship to assist in, among other things, the following: (a) alleviating mental or emotional illness, symptoms, conditions, or disorders, including alcohol or drug addiction; (b) understanding conscious or subconscious motivations; (c) resolving emotional, attitudinal, or relationship conflicts; or (d) modifying feelings, attitudes, or behaviors that interfere with effective emotional, social or intellectual functioning, and in the event such advice is given concerning these problems of the like, such advice is not given in a professional capacity, but is given by the above named only in his/her recognized capacity as a biblically-based Pastoral Care, without any further representation of professionalism.
I acknowledge and understand that I take full responsibility for my decisions after this Pastoral Care session.
I have fully informed myself of this Pastoral Care Acknowledgment and Consent by reading and understanding it before signing it.
Confidentiality is an essential aspect of the Pastoral Care relationship, and we will carefully guard the information you entrust to us. All communications between you and our Pastoral Care Ministry team will be held in strict confidence unless you (or a parent in the case of a minor) give the authorization to release this information.
The exceptions to this include: 1) If a person expresses intent or desire to harm himself/herself or someone else; 2) If there is evidence or reasonable suspicion of abuse or neglect of or against a minor child, elder person, or adult. Our biblical counselors strongly prefer not to disclose personal information to others; they will make every effort to help you find ways to resolve a problem as privately as possible.
